Department Overview
The Mental Health Transition Office creates seamless pathways for students returning to school following mental health
treatment. Our specialized team bridges the gap between clinical care and educational settings by coordinating with
mental health treatment providers, families, and school personnel to ensure successful reintegration into the learning
environment.
Coordinated Care
- Facilitating essential communication between hospitals, treatment centers, parents/guardians, and school teams
- Analyzing discharge documentation to translate clinical recommendations into practical educational supports
- Connecting families with appropriate community mental health resources to ensure continuity of care
Educational Support
- Guidance for developing comprehensive reentry strategies tailored to each student’s specific needs
- Providing consultation to school teams implementing accommodation plans
- Ensuring students receive appropriate academic and emotional support during transition periods
Professional Learning
- Training school personnel in evidence-based reentry planning techniques
- Equipping educational teams with skills to support students following mental health treatment
- Building capacity within schools to maintain supportive environments for all students
